How does Baricent 4 work?

Baricent works by blocking the release of certain enzymes (JAK1 and JAK2 enzymes) that increase pain and swelling. Thus, this medicine provides relief from pain and reduces discomfort in your physical movements. 

Side Effects of Baricent 4

• Common adverse reactions include:
  • Increased LDL cholesterol (33.6%)
  • Upper respiratory tract infections (14.7%)
  • Nausea (2.8%)

• Baricitinib is used in the treatment of RA.
• It can be taken with or without food as advised by your doctor.
• Do not take more than the prescribed dose.

• Inform your doctor if you have liver, kidney, or lung problems or any disease like hepatitis, or active infections.

Indication of Baricent 4

Baricitinib is used in the treatment of rheumatoid arthritis.

Pharmacology

Baricitinib is a selective inhibitor of Janus kinase 1 (JAK1) and Janus kinase 2 (JAK2). These enzymes are involved in signaling pathways that regulate immune and inflammatory responses. By inhibiting JAK1 and JAK2, Baricitinib reduces the activation of signal transducers and activators of transcription (STATs), which leads to decreased inflammation and immune activity in rheumatoid arthritis.

Dosage & Administration of Baricent 4

• Adult Dose: 2 mg once daily, which can be used as monotherapy or in combination with Methotrexate or other DMARDs.
• Pediatric Use: Not established for use in pediatric patients.
• Geriatric Use: Careful dose selection is advised in elderly patients due to the potential for reduced renal function.
• Hepatic Impairment: No dose adjustment is necessary for mild or moderate hepatic impairment.
• Renal Impairment: Baricitinib is not recommended in patients with an estimated GFR < 60 mL/min/1.73 m².

In case of overdose, monitor the patient for adverse reactions and provide appropriate treatment if needed.

Interaction

• Strong OAT3 Inhibitors: Co-administration with strong OAT3 inhibitors (e.g., probenecid) increases Baricitinib exposure.
• Other JAK Inhibitors or Biologic DMARDs: Not studied in combination with other JAK inhibitors or biologic DMARDs.

Contraindications

• Anemia: Avoid use in patients with hemoglobin levels below 8 g/dL.
• Lymphopenia: Do not initiate or continue Baricitinib if the absolute lymphocyte count is below 500 cells/mm³.
• Neutropenia: Avoid starting or continuing treatment if the absolute neutrophil count is below 1000 cells/mm³.

Pregnancy & Lactation

• Contraindicated during pregnancy.
• No data is available on the excretion of baricitinib in human breast milk.

Precautions & Warnings

• Serious Infections: Avoid in patients with serious active infections, including localized infections.
• Tuberculosis: Do not administer to patients with active TB.
• Malignancy & Lymphoproliferative Disorders: Weigh risks and benefits in patients with known malignancies.
• Thrombosis: Use with caution in patients at increased risk of thrombosis.
• Gastrointestinal Perforations: Exercise caution in patients with an increased risk of GI perforation.
